What is the most expensive technical problem you have found on a site?

Every technical SEO has one story where a single setting was costing a business real money: a robots rule shipped from staging, a canonical pointing at the wrong page across a whole template, a redirect chain nobody noticed for a year.
Two things are interesting about those stories. First, what the problem actually was. Second, how it was found: whether a crawler caught it, whether it showed up in Search Console, or whether someone happened to look at the page in a private window on a phone.
So: what was yours, and what found it? If you can say roughly what it cost in traffic or revenue, even better, because that is the part that gets technical work prioritised.
